Not a zip-code - the 36 is the STREET-address/number (like in house 36, in this case Flat 36), and the 7 is the APARTMENTnumber in that flat.
This kind of numbering is quite common because a lot of people live in flats.
Do note Oleg's hint for the zip-code - that's the only thing missing in your address above.
36/7 its a number of house. number of appartment we write like this:
Ivanova str 23/1, app 21
"/" It means that the house in a court yard. Or between old houses have constructed new
in russian:
Украина, Черновцы, ул. Заслонова 36/7
I try to find this street on the online map of Chernovtsy but i cant,it didnt exist. Can it is not present in a map
