Having overcome one obstacle, there is now another. My fiancee now has her passport, but we are having trouble getting one for her daughter due to an apparent shortage of children's passports. My fiancee lives in Lugansk. Is this for real? Well, I telephoned a couple of friends in other parts of Ukraine, and yes, their passport offices too have just a handful of blank childrens' passports and there is a long waiting list for them. I have seen discussion on the web about this before.

I am sure i can help you. Just give more information: date of birth of the kid; the perpose of leaving Ukraine; has his father agreed on leaving Ukraine by his kid? After this i provide you with some advises tomorrow.
In order to leave Ukraine the child need to have a Travel Document and the Passport (if the child is older than 16).
An application on issuing the Travel Document shall not be considered by the state authority for more than 3 months upon the day of submitting the application.
Among other documents submitted for obtaining the Travel Document you shall submit:
- in case of leaving Ukraine by the child with one of the parents (as your case is) - special application of the father where he has not any claims to the child's mother regarding unpaid alimony.
- in case of such a leaving for permanent residence you shall submit the father's written and notarized agree on this too.
In addition, information about the child shall be entered into the mother's passport upon her application.
Issuing of the travel document is drawn up upon the notarized application of the parents. If the parents are divorced then such an application shall submit those on who lives with the child (your woman as I understand). If the father does not agree on his child leaving Ukraine, the other one can get this agree from court.
In case the child is under 16 years, the Birth Certificate shall be submitted to the territorial body or subdivision, diplomatic representatives or consular office of Ukraine and the Ministry of Foreign Affairs (MZS in Ukrainian). If the child is between 16 and 18, then the Passport of Ukrainian citizen shall be submitted instead of the Birth Certificate.
That is all for now. Tomorrow I will write the main extracts of how to obtain the Passport. Anyway specify your problem and remember that they cannot delay the term of issue of Passport that is fixed by the Law.
The Passport is issued by the territorial subdivision of citizen affairs, immigration and registration of natural persons of municipal, district offices (departments) of the Ministry of Internal Affairs (MVD in Ukrainian) (further in the text - the Territorial subdivisions) according to the place of an applicant's living upon he is 16 years old.
The Passport shall be issued and transferred to the applicant within one month.
The application and other documents shall be submitted to authorized persons of the municipal housing economy (ZhEK in Ukrainian), the Municipal State Administration of Kyiv or Sevastopil cities, village or settlement councils (and in special cases some others that specified in the Law).
Those who live in private houses or apartments that are not governed by municipal authority shall submit the application and other documents directly to the Territorial subdivisions.

I made a short list of the most important items. The hole lists of the documents and the procedure you may find in the relevant legislative acts. I cannot image how they do not issue the Passport or Travel Document to the child. This may have place only if your documents are with defects. In such a case they must explain to you what is wrong.

Some advices from my experience:
- you always can find the procedure and the requirements on special information boards at the relevant authorities;
- it is always good to go or call there and ask to be sure. This is because a responsible officer may has "his own viewpoint". It is not always possible to talk with them because this is not their obligation, but try anyway.
- Permanently control the situation. If you get to know that your application is denied then you may have chances to speak with the officer and maybe to settle an issue. Otherwise, after they deny, you will need to start the procedure from the beginning and wait one more month.
- Ukraine is not jungles. To issue the Passport and others is the obligation of the authorities. So if they delay terms or unlawfully denied, this is high time to claim.
And I would not suggest you bribes. A bribe means illegal. Thus every interested person can contest the decision for which you paid. And its cancelation is not the worst thing that may happen. At the same time it is possible (but must be done very accuracy) to stimulate more swift response, to give a chance to correct a defected document, etc.
Hi Pitter Pen, I really appreciate you going out of your way to help. My fiancee's daughter is 14 years old. My fiancee has a passport. Her former husband has signed a notarised document saying: "I, ..., am aware of the application of my former wife concerning departure abroad together with my child ... I declare that I have no objections for their departure abroad for permanent residence and that (my fiancee) has no unfulfilled obligations and property duties towards me ...".
He has now agreed to co-operate with signing any other required documentation. The problem therefore seems to be purely the availability of "blank" childrens' passports. There appears to be a shortage of them in both Lugansk and Dnepropetrovsk and my fiancee, I am told, is limited to getting this passport within her own jurisdiction of Lugansk anyway.
Undoubtedly GeminiDreams is right -- a shortage opens the door for bribery but I'd prefer to find another way, where possible. What would you recommend? Thank you.
I am confused, does a child under age of 16 need a passport to leave country?
Chris, it sounds like you have the travel document with a statement that child can leave country from the father.....isn't that all you need? PitterPen mentioned that a couple posts above.....
I've spoken with my fiancee again and clarified a few things. As her daughter is 14, we are, as Pitter Pen asserts, talking about a travel document -- not a passport. Her daughter apparently does not need a passport to exit the Ukraine, only a travel document (although I'm yet to confirm whether this is sufficient for her entering Australia). My fiancee's problem is NOT about compiling the necessary documents. She is a master at this, given the effort and time she has spent preparing documents for her visa already. The shortage she refers to is of travel document 'blanks'. Nevertheless, today she is compiling all the necessary paper work and she will formally apply for the travel document even though she has been told they are in short supply. ((My fiancee, tongue in cheek, has described putting in this application as like 'ploughing the sand'.)
Pitter Pen, the officials keep saying that they haven't any "blank" travel documents, even if her daughter qualifies to leave Ukraine, which she does. So what can we do to get this document, short of bribing the officials -- and I agree, I don't want to go down this path of bribery which is all too much a way of life in Ukraine bureaucracy.
Beemer18 is right. Above i wrote about the Travel Document. Your child DOESN'T need and CANNOT have the Passport because he/she is only 14. You need to obtain only the TRAVEL DOCUMENT and some other.
Lets make it clear. As i understand you have not problems with any other document except the Travel Document. And the officials say that they just do not have blanks. REMEMBER, this is not a reason to break the 3-month term (see my post about the Travel Document above). But knowing my Goverment i can believe in this.
Have you submitted all necessary documents to the authority? In advance i will write how it is needed to be done. Prepare all needed documents and make the list of them (in two copies). Submit the documents and the list to the receptionist of the relevant office. The receptionist MUST affix his/her full name, the date of receiving the documents, sign the list, affix a reference number and affix the stamp (if it is possible). This copyof the list you MUST keep till you get the Travel Document. It is an evidanve of submitting the application and the date of the application. The other copy you may leave at the receptionist if he/she wants. After this the 3-month term begins. You may also send the application by mail using a specail type of letter – Letter with a notice and an enclosure. The result will be the same, you will get the proof of submitting the application on a certain date. Also may you fiancée go to the office (to the Chief officer) and ask him/her when she can get the Travel Document showing the list or certificate from the Post wich indicates that the office got the application. If they still complain that they do not have blanks, may you fiancee ask them how they are going to resolve the problem. Sould they do not know I presume three variants:
1) you may wait and maybe they will get the blanks and issue the Travel Documents within 3 months;
2) they will not and you will need to file a claim to court (it will take one month or more to get the decision);
3) you may stimulate the officer.
You see, they will issue the Travel Document to you in any case. The question is when. So if you need to get the Travel Document as soon as possible I recommend you the option three. This is not actually a bribe. But anyway I would not like to write here about it. If you like we may discuss it via private messages.
